Yes. So far as I know it fits all 395's.

It fit in the 275 tool that I own so I would say yes. I understand that there were several different versions of the 275 but I believe the coupling was used in most of the Dremel tools.

Does this coupling have a plastic gear inserted on one end?

Not as supplied. It slides over what might look like a gear on the motor and spindle, but they don't come with this coupling.
