The remaining comments are still valid (no install discs, box, etc.) but the RAID controller probably was fine, as the replacement one I got from SCSI4ME worked fine after I corrected a BIOS setting I had made while trying to debug my issues and that actually caused the failure to boot problem. I bought this to replace the same controller in my home server that had failed. Therefore, I had the software and drivers already installed, which is a good thing since nothing other than the card came with the order. If this is new (as advertised), then it was removed from the packaging and placed in a ziploc bag, which is how I received it. The new one I bought that this was to replace came in a box and install CD as well as an installation manual. This had none of them. I was working with Broadcom to try to figure out how to recover my old RAID information so as to recover my array so it took a week or so of back and forth with them to find out they couldn't really help. They told me that the card was out of warranty, which had the card been new, would not seem correct. However, the price was about 30% of the price I paid years ago for the new one. When I finally went to set up the RAID, my computer would not boot (got stuck in a AE code (ASUS motherboard), which indicated it was hung on a QPI initialization error.
