I forgot to pull the speaker grille off the old screen, so I had to remove and reinstall the screen a second time. I suspect it was this reinstall where I added the blemish. The included 3M double sided tape does the job, but it is very difficult to remove from the sheet and place. I doubt the phone is anywhere near the IP68 waterproof rating that it shipped with.
