Yes it did come with a carrying case

After less than a month of use I keep getting Er5. I looked in the manual and googled around and can't seem to find out what Error 5 is. Anyone ideas?

After a few months of using mine I got Error 5 also. I tried miniscule adjustments to the insertion of the test strip and it worked (the most common adjustment was slightly lifting up on the end I was holding so it seemed to push the far corner of the strip more into the slot). I just now tried it again and it worked without any adjustment needed (I don't use it every day, only once in a while). Hope this helps.

Does this come with the device and needle stick pen?

No stick pen. Personally, I've got lots of those,anyway.
