Ours broke inside the handrail as well. Any ideas on how to get it out?

Sorry to hear the bolt snapped. You said it broke in the rail but I assuming you meant the newel post? If you did not put some wax or other lubricant on the bolt and reamed the hole a bit using the proper size bit in hardwood it is possible that the bolt could fail. Assuming it is snapped in the newel the best way is to take a small wood chisel and carefully carve in a bit all around the bolt to try and leave enough sticking out to grab with locking pliers. Can this be used on both enda of tge handrail

Yes. I installed one at each end of our oak handrail. Things got a bit tight at the upper end, where a spindle had to be installed in the same spot, but a liberal amount of PL-400 solved the problem. By the way ... these bolts are marvellous!

Are oak plugs included?

There are no oak plugs included in this bulk pack. The Zipbolt 11.560 is the same bolt with plugs included.
