Would these Neoprene straps hold up well when playing basketball?

These straps would not hold up well under any circumstances. Way too short and they barely fit around the back of my head so not enough of the strap to slide onto glasses frame securely. Look for something different.

For size, what is difference one size or 1pack

It means ..chum comes in one size...one to a pack...that's how mine came...

Can a strap be fed all the way through the retainer?

Good question! Sorry it's been ignored. No, closed-cell foam fills the center portion so you can't (easily) thread a cord through for different attachment options. You can buy closed-cell foam insulation (for refrigerator tubing) with a narrow inside diameter and no longitudinal slit through which you can easily thread any cord with any terminal hardware, whatever works best for you. A bit of that will float far more mass than these neoprene retainers. Unfortunately, I've only ever seen it in black.
