where is it made? what country?

The packages I got were made in India. That is not a guarantee where all future ones will also be made. India has a large drug manufacturing industry, especially for common non-prescription drugs.

Does this work on toenail fungus?

Some fungi for toenail fungus are in the tinea family, but others are from bacteria. While miconazole treats some tinea fungi, it would be best to contact a physician, as toenail fungi will be embedded within the nail, and not accessible to the cream.

I am thinking it is made in India from what I remember.
