Can I turn the handle once I get in the tub to help me get back out of the tub?

Yes, you can. You turn the circular knob, rotate the handle grip, and turn the knob to tighten.

Can the handrail be installed in a spa already filled with water?

Yes, I installed the handrail on a filled system. I used a mallet to drive the base under my filled hot tub. You'll have to exert some energy to get it done! It took me about 10 minutes.

Is this wobbly , even with base attached to deck?

Yes, it is... I have mine screwed (with extra screws) to concrete. It is secured but the mounting plate here is thin so it gives a bit at the base regardless of how well it is secured to any base. And although it's unlikely going to tip over, it doesn't give the sense of safety when someone grabs hold of it to help them.
